Cornerstone Liverpool Kids' Church Registration (Creche and Kids' Church)

The form below ensures we have the relevant consents and any important information from you as their parent/carer to ensure that your child(ren) have a safe and enjoyable time at Cornerstone Kids. We will ask you to complete a new form occasionally to help us ensure we have up to date information on you and your children.

If anything changes regarding you/your children that we need to know about (in particular related to medical or dietary needs, contact details etc.) please complete a new submission as soon as possible (contact debbie@cornerstonechurchliverpool.org if you need the form link sending to you).

Finally, we always endeavour to take every reasonable step to keep your child(ren) safe whilst they are in our care at Kids' Church/Creche, but please be aware that when your child(ren) return to you towards the end of the church service, parents/carers are then responsible for the care and behaviour of your own child(ren).

Emergency Consent *

I give consent for my child to be released to this person in the event of emergency or illness, if I cannot be contacted or am unavailable.

Emergency Medical Consent *

I agree for my child to be given first aid by a trained member of the Kids Team if necessary. This includes applying plasters and the administering of any medication as specified above. If I cannot be contacted and in the event of an emergency I consent for my child to receive urgent dental, medical or surgical treatment, including anaesthetics, as may be considered necessary by the medical authorities present. My child's information may also be shared with the NHS and/or other relevant health professionals in such cases.
