Expression of interest in serving on a ministry team (CCW)

Please complete this form so that we can gather the information we need about our volunteers. In addition to this, we would like all volunteers on all teams to complete a DBS check for the protection of both yourselves as volunteers, and for the children and adults in our church.

If you are volunteering for a ministry area which involves working with children, we would ask that you provide the contact details of two people in the form who can provide a character reference for you, these details will be followed up on by one of the Safeguarding team.
